The tooth pulp can become damaged or affected due to many reasons. Reasons such as decay, trauma, large filling, extensive gum disease and even drilling or grinding a tooth may result in the tooth pulp to be affected. 

When this occurs the tooth will need root canal therapy to prevent the spread of its aftermath to the surrounding structures.

There are cases were the patient feels no pain, this is when the pulp is necrotic or dead and could have a relief valve for drainage, also known as fistula.

It is not uncommon for patients to suffer pain after root canal therapy that should not last more than 3-4 days, but it is controlled with painkillers and not antibiotics.
